Whole Life Health Care is administering the Moderna COVID-19 Vaccine (2023-2024 Formula) to prevent Coronavirus Disease 2019 (COVID-19) for use in individuals 6 months through 11 years of age.
COVID-19 Vaccine for 6 mo – 11 yrs:
– 1st shot
– 2nd shot (at least 1 month after 1st shot) if pt has had first shot
Please call the office for appointment availability. 603-431-6677. Phone hours are 9:00 am – 12:30 pm and 1:45 pm – 4:45 pm. Anyone who answers the phone can schedule an appointment for your child.
You will be required to complete an online appointment registration which includes a COVID-19 Vaccine questionnaire. Please advise the staff if you need to complete the registration on paper instead of electronically.
