How To Become A New Patient

If you are interested in becoming a new patient of the Family Practice, please see below.




  • To become a new patient:
    • Please note that we do not accept MaineCare or Mass Health.  We are unable to accept you as a new patient if MaineCare or Mass Health is either your primary or secondary insurance.
    • Please confirm that Amy Coombs, APRN is in network for your insurance plan
    • Determine when your last annual physical was and when your next appointment is due (and what it is for – follow-up, physical, med check, etc.)
    • Please email newpatient@mywholelifehealthcare.com with the following information:
      • First and last name
      • Phone number
      • Name of insurance (i.e., Anthem BlueCross BlueShield, self-pay, etc.)
      • Please identify yourself as either a former patient of Anna Pilzer, Jessica Roundy or Nicole Rembisz or a new patient to WLHC
      • Let us know if you would be interested in having your first appointment scheduled as Telemedicine (must have video capability).
        • NOTE:  WLHC does not have encrypted email.  Please keep in mind that communications via email over the internet are not secure. Although it is unlikely, there is a possibility that information you include in an email can be intercepted and read by other parties besides the person to whom it is addressed.  By emailing us, you accept these risks.
    • New patient registration will take place over the phone.
      • Please keep your insurance information handy in anticipation of our call to register you as a new patient.
        • If you do not have your insurance information available, we will be happy to register you and will require a call back to update your insurance and schedule your first appointment.
        • We may also require a photo of the front and back of your insurance card if your insurance is not showing as eligible in our system.

  • If you are a former patient of Anna Pilzer, APRN, Jessica Roundy, APRN or Nicole Rembisz, APRN:
    • Your first appointment will be scheduled based on provider availability.
  • If you are coming from a different provider’s office:
    • Your first appointment as a new patient will be an office visit. It will be subject to a co-pay, co-insurance or deductible, depending on your insurance plan.
    • First appointment availability varies, depending on which provider is requested.

All New Patients: 
You will receive a text and an email reminder 5 days before for your appointment, from which you will complete the registration process through online check-in.  You can complete the online check-in on a smart phone, tablet or computer.

If you don’t have access to a computer or the internet, please let us know at the time we schedule your appointment.  We will set you up to complete this process in the office and ask that you arrive 30 minutes before your appointment time to allow plenty of time to complete the New Patient paperwork.
