How To Become A New Patient

If you are interested in becoming a new patient of the Family Practice, please see below.




  • If you are a former patient of Anna Pilzer, APRN:
    • We will begin to register Anna’s former patients on November 1, 2022
    • Please note that we do not accept MaineCare
    • Please confirm that Amy Coombs, APRN is in network for your insurance plan
    • Determine when your last annual physical was with Anna and when your next appointment is due (and what it is for – follow-up, physical, med check, etc.)
    • On or after November 1, 2022, please call Kelly Bailey at 603-431-6677 x338 or email kbailey@mywholelifehealthcare.com with the following information:
      • First and last name
      • Phone number
      • Name of insurance (i.e., Anthem BlueCross BlueShield, self-pay, etc.)
      • Please identify yourself as a former patient of Anna Pilzer
    • New patient registration will take place over the phone
    • We look forward to speaking to speaking with you!

  • If you are a former patient of Nicole Rembisz, APRN:
    • Please confirm that Amy Coombs, APRN is in network for your insurance plan
    • Determine when your last annual physical was with Nicole and when your next appointment is due (and what it is for – follow-up, physical, med check, etc.)
    • Please call Kelly Bailey at 603-431-6677 x338 or email kbailey@mywholelifehealthcare.com with the following information:
      • First and last name
      • Phone number
      • Name of insurance (i.e., Anthem BlueCross BlueShield, self-pay, etc.)
      • Please identify yourself as a former patient of Nicole Rembisz
    • New patient registration will take place over the phone

  • If you are coming from a different provider’s office:
    • Please confirm that Amy Coombs, APRN is in network for your insurance plan
    • Please call Kelly Bailey, our New Patient Coordinator, at 603-431-6677 x338 or email kbailey@mywholelifehealthcare.com with the following information:
      • First and last name
      • Phone number
      • Name of insurance (i.e., Anthem BlueCross BlueShield, self-pay, etc.)
      • Please identify yourself as wanting to establish care as a new patient
    • Please be advised that:
      • New patient registration will take place over the phone
      • Your first appointment as a new patient will be an office visit. It will be subject to a co-pay, co-insurance or deductible, depending on your insurance plan.
      • First appointment availability varies, depending on which provider is requested.

All New Patients: 
You will receive a text and an email reminder 4 days before for your appointment, from which you will complete the registration process through online check-in.  You can complete the online check-in on a smart phone, tablet or computer.

If you don’t have access to a computer or the internet, please let us know at the time we schedule your appointment.  We will set you up to complete this process in the office and ask that you arrive 30 minutes before your appointment time to allow plenty of time to complete the New Patient paperwork.
