Appointment Check-In Process

Prior to your appointment, please log onto the patient portal and check-in.

  • Log in at
  • Your appointment will be displayed at the top of the home page.
    • Click the blue “Check-In” button
    • Do I pay my co-pay online?
      • NO if it’s a physical or well child check
      • YES if it’s any other type of appointment

Effective Friday, December 4, 2020, Whole Life Health Care is asking all patients to remain in your car and call the office when you arrive. We will screen you for COVID and check you in over the phone.

COVID Screening:
If you have any of the following symptoms or answer yes to the questions, we will need to convert your appointment to telemedicine, if appropriate, or reschedule for another day:

Fever or chills                                                                         Headache
Cough                                                                                       New loss of taste or smell
Shortness of breath or difficulty breathing                      Sore throat
Fatigue                                                                                     Nausea or vomiting
Muscle or body aches                                                           Diarrhea

We will also want to know if over the past 10 days you have:
– Traveled outside the New England states (ME, NH, VT, MA, RI, CT)
– Attended any recent social gatherings which included anyone outside your immediate household?
– Been in contact with anyone you know is positive for COVID

Please remain in the comfort of your car after you’ve been checked-in.  Your nurse or Integrative Provider will then call or text you when they are ready for you.
