How To Become A New Patient

If you are interested in becoming a new patient, please check with your insurance to see if Amy Coombs, APRN, is in network with your plan. If she is, all of our providers will also be in network.

 
Once you have confirmed we are in network for your insurance, or if you will be a self-pay patient, please contact our office by phone or email. You are welcome to call the office at 603-431-6677 and ask to speak to our New Patient Coordinator. You may also email us at info@mywholelifehealthcare.com. Our New Patient Coordinator will need your name, phone number and email address to contact you about becoming a new patient.

During our onboarding process, you will be asked to complete a New Patient Registration Form. Upon receipt of the completed form, you will be contacted within 5 – 10 business days.

Your first appointment as a new patient will be an office visit. It will be subject to a co-pay, co-insurance or deductible, depending on your insurance plan. First appointment availability varies, depending on which provider is requested.
